Indiana State Medical Association Breach: The IT chief of the ISMA, of all people, lost a laptop and two hard drives containing Social Security Numbers and medical information of about 40,000 beneficiaries of the Association's health insurance plan.  Apparently they were (i) unencrypted and (ii) left in plain sight in his unlocked car.  In addition, he failed to report the theft for 24 hours.

Possible upside: ISMA's insurance is through Anthem, so maybe all of the data had already been stolen when Anthem got hacked.
